• Data is currently being gathered to establish national norms for several rating scales:
• All of the materials are supplied free-of-charge.
• Forms are completed by educators AND/OR the parents/guardians of children ages 2 through 18 years.
• An individual’s identity will not be used in the rating process and all information will remain completely confidential; data will be reported at the group (i.e., age, gender, etc.) level only.
  
• Three dollars ($3.00) will be paid for EACH form properly completed and returned to this company.
• Participation requires a minimum of 10 forms be completed and returned for compensation.
