Ages 5-17

The Attention Deficit Disorder Evaluation Scale-Fifth Edition (ADDES-5) enables educators, school and private psychologists, pediatricians, and other medical personnel to evaluate Attention-Deficit/Hyperactivity Disorder in children and youth from input provided by primary observers of the student’s behavior.

The ADDES-5 was developed from research in behavior disorders, learning disabilities, and Attention-Deficit/Hyperactivity Disorder; current literature in psychology, neurology, and education; and current DSM-5™ practices in identification and diagnosis. The subscales, Inattentive and Hyperactive-Impulsive, are based on the current characteristics of ADHD. The results provided by the scale are commensurate with criteria used by educational, psychiatric, and pediatric professionals to identify Attention-Deficit/Hyperactivity Disorder in children and youth. The scale is available in two versions: School Version, a reporting form for educators, and Home Version, a reporting form for parents.

Internal consistency, test-retest, and inter-rater reliability; item and factor analysis; and content, diagnostic, criterion-related, and construct validity are documented and reported for the scale.

Characteristics and Components of the ADDES-5

  • The ADDES-5 is based on the DSM-5™ definition of Attention-Deficit/Hyperactivity Disorder and the criteria most widely accepted by educators and mental health professionals.

  • The ADDES-5 School Version was standardized on a total of 4,829 students, including students identified with Attention-Deficit/Hyperactivity Disorder.

  • The standardization population included students from 23 states and represented all geographic regions of the United States.

  • Factor analysis confirmed the two ADDES-5 subscales.

  • The ADDES-5 provides separate norms for male and female students 5 through 17 years of age.

  • The School Version can be completed in approximately 15 minutes and includes 60 items easily observed and documented by educational personnel.

  • The Home Version can be completed by a parent or guardian in approximately 12 minutes and includes 46 items representing behaviors exhibited in and around the home environment.

  • The Pre-Referral Attention Deficit Checklist provides a format for identifying behaviors in need of intervention.

  • The Attention Deficit Disorder Intervention Manual - Revised includes IEP goals, objectives, and interventions for all 60 items on the School Version of the scale.

  • The Parent’s Guide to Attention Deficit Disorder provides parents with specific, practical strategies to use in helping their child be more successful in the home environment.

  • The ADDES-5 Quick Score computer program converts raw scores to standard scores and percentiles which makes scoring efficient and convenient.

  • Spanish language Home and School Version Rating Forms are available.

Educational Planning

The Attention Deficit Disorder Intervention Manual - Revised (216 pages, © 2021) offers intervention strategies to help professionals manage 60 of the most common behavioral characteristics of students with Attention-Deficit/Hyperactivity Disorder. The manual includes goals and objectives for an IEP, as well as a comprehensive collection of intervention strategies to help students be more successful in the school environment.

Parental Involvement

The Parent’s Guide to Attention Deficit Disorder (126 pages, © 1995) provides logical and useful suggestions for parents searching for solutions to their child’s problems. The user-friendly format has hundreds of suggestions for parents to help their child who has an Attention-Deficit/Hyperactivity Disorder be more successful. The guide includes all behaviors from the Home Version of the Attention Deficit Disorder Evaluation Scale - Fifth Edition.

SOFTWARE REQUIREMENTS

  • Windows®-compatible Quick Score computer programs require Windows® 2000 or newer, and 10 MB of RAM, 10MB of available hard disk space.
